Health Questionnaire & Test Results
Register via the link below to receive a health questionnaire from your doctor. The questionnaire helps us better understand your needs, and you’ll get a notification when it’s ready to answer. You’ll also find your test results here once they’ve been analyzed, along with personalized comments from your doctor.

Leave Blood Work
Once your health questionnaire is completed, your referral will be activated. You can book an appointment or use drop-in at any of these testing locations.
Please note that this is a fasting test. You should refrain from both food and drink from 8:00 PM the evening before your test.

As soon as we have received your health questionnaire and lab results, we will contact you to schedule your consultation with a physician. During the consultation, we will review your test results.
